• Patient Registration Form • Payment Policy Acknowledgement • Privacy Practices Acknowledgement • Records Release Form • Health History and Medication Form |
To expedite your appointment please bring a copy of your last eye exam, or make sure we have your records release form signed one week prior to your exam so that we may obtain records on your behalf.
